As a patient or caregiver, you can use the MyHealth360 mobile app to access:
- Radiology reports
- Laboratory reports
- Health screening reports
- COVID-19 test results

How it works
During the clinic visit, we will capture your mobile number. We will use the number to send an SMS notification once the results are available in the MyHealth360 app.
Medical Records Office (MRO)
Alternatively, you may approach our Medical Records Office for assistance. Before you contact the MRO, please prepare the following documents:
- A Consent for Release of Medical Information (PDF) from the patient to release the medical information.
- A copy of the patient’s and authorised requestor’s NRIC or passport for verification.
- (If the patient is below 21 years old) The consent of the patient’s parents or legal guardian (PDF).
- (If the patient is deceased or lacks mental capacity) The consent of the patient’s legal representative (PDF). Where relevant, a copy of the death certificate may be required.

Please allow a minimum of 5 working days for the processing of discharge summaries and Radiology/Laboratory results requests. Requests for other medical information will be assessed on a case-by-case basis, and applicable administrative fees may be charged. You may receive your requested medical records through email, mail or self-collection at our MRO.
